Working with the Quality Improvement Organization:  What has Changed and What Nurses and Administrators Need to Know
A Quality Improvement Organization (QIO) is a group of health quality experts, clinicians, and consumers organized to improve patient care and review quality care issues for the Medicare population through a variety of ways. August 1, 2014 there were extensive changes to the QIO programs managed by CMS and the contractors providing these services. Quality Assessment and Performance Improvement (QAPI):   The Impact of Effective Clinical Practice, Processes, and Workflow  to Improve Patient Care and Satisfaction

Quality Assessment and Performance Improvement (QAPI) is a state and federal requirement for home health and hospice agencies. The problem with the requirement is that it has become a robotic process of chart and progress note review leading to corrections being made to the documentation and never identifying the true reason that the errors are occurring.  This process can be identified as a “bandaid” philosophy and can set an organization up for possible compliance issues. Effective QAPI involves members at all levels of the organization and is a data-driven approach that identifies areas of concern in clinical practice, workflow, and/or processes that have a potential of compromising patient care. The QAPI team does an in-depth review of the identified gaps, and develops and implements appropriate corrective action plans with continued monitoring for effectiveness of interventions. Diabetes is becoming more prevalent in the older population due to increased life expectancy. According to the Center for Disease Control and Prevention (CDC) in 2010, 25.8 million people, or 8.3% of the population had diabetes and 10.9 million, or 26.9% of those were 65 years or older. Diabetes is associated with multiple co-morbidities such as hypertension, hyperlipidemia, cardiovascular disease, kidney disease, nonalcoholic fatty liver disease, obesity, and vascular complications. A wound or ulcer to the skin can be caused by disease process, injury, or a surgical procedure.  As a person ages, the skin becomes thinner with less moisture and elasticity and the body’s ability to heal can be decreased. Wounds can be considered acute or chronic.  Acute wounds can include abrasions, lacerations, punctures, or incisions. Chronic wounds can include venous, peripheral arterial, diabetic and pressure ulcers. Most wounds, but especially chronic wounds can cause extreme stress to patients and result in increased expense to care for with multiple products, long treatment time, and hospitalizations. Effective management of wounds requires understanding the goal for the patient.  Although a curative approach for wound care is the norm that is seen in most situations, there are times at end of life that palliative wound care is also appropriate. Each year millions of adults 65 years or older experience falls resulting in moderate to severe injuries.  According to the Center for Disease Control and Prevention (CDC) one out of three older adults fall and less than half talk to their healthcare providers about it.  CDC also reported in 2013, that 2.5 million non-fatal falls among older adults were treated in an emergency department with more than 734,000 of those patients hospitalized. These statistics are overwhelming, as most falls result in further issues such as reducing the ability to live independently and even death. An underserved patient is less likely to get healthcare when it is needed due to a multitude of reasons.  Affecting factors can be age, language, race, education, socio-economic status, mental health disparities, lack of insurability, and/or race. Utilizing and Understanding OASIS/CASPER Reports to Effectively Manage Your Home Health Patient Population
Medicare-certified home health agencies are required to use a standard set of data items, known as OASIS (Outcome and Assessment Information Set) as part of a comprehensive assessment for all patients receiving skilled care that is reimbursed by Medicare or Medicaid. OASIS data sets are collected at specified time points and CASPER reports are generated for an agency to review and use for quality improvement in issues identified. Understanding these reports and how to use them to make appropriate changes to clinical practice and processes can be a challenge.
