The first mandatory Alternative Payment Model: the Comprehensive Care for Joint Replacements (CCJR) Initiative, will create Episodic Bundling for Total Joint Replacement patients as soon as April 1st, 2016. Based on the CMS Innovation Center BPCI Model 2 Pilot Programs, CCJR makes the anchor hospital clinically and financially responsible for patient treatment and costs for 90 post-discharge. Rewiring care is mandatory for success in this model, and even the most progressive systems have work to do to create successful programs for CCJR Bundling requirements. In addition, all Medicare discharges, regardless of diagnosis, will be Bundled by 2018, making CCJR a trial run for the care design of tomorrow.
