Audio Conference: Brand New Standards for Case Managers/Discharge Planners in All Practice Settings

When:  Nov 10, 2016 from 12:00 PM to 01:30 PM (CT)
The Case Management Society of America (CMSA) has published revised national standards of care that govern the practice of case management/discharge planning.  These standards apply to all practice settings, including hospitals, skilled nursing facilities (SNFs), home health agencies, assisted living facilities (ALFs), private duty agencies, hospices and HME companies. A key emphasis in the healthcare industry today is collaboration, which must include effective case management, especially for care transitions.  Join this teleconference to find out what case managers/discharge planners are now required to do and the standards that will likely be applied to them in disciplinary procedures before state licensure boards.   Since these standards are time-consuming and demanding to meet, what can case managers/discharge planners do to effectively meet them, especially when resources are scarce?  What legal and ethical aspects of the standards are important for case managers/discharge planners to understand?  The emphasis during the teleconference will be on practical strategies to provide effective case management services.
